We are accepting new patients and look forward to welcoming you. Here is everything you need to know before your first visit.
What to Bring
To make your first visit as smooth as possible, please bring the following items with you to your appointment.
- Photo ID (driver's license or government-issued ID)
- Insurance card(s)
- List of current medications and dosages
- Previous eyeglass or contact lens prescription, if you have one
- Names of any other eye care providers or physicians you see
- Any relevant prior medical records or imaging

To make an appointment, please call our office and our friendly staff will find a time that works for you.
Insurance & Payment
We are providers for medically-based eye care for most major insurance plans. Please contact our office to confirm your specific plan is accepted. Several plans require a referral or prior authorization — we are happy to assist you with that process.
Plans We Accept
Medicare
Medicare covers an eye examination or office visit when there is an underlying medical condition such as glaucoma, macular degeneration, diabetic retinopathy, or other medically necessary indications. Routine refraction is not covered by Medicare.

- ›Is EyeCare Chattanooga listed as a Participating Provider on my plan?
Payment Methods
If you do not have insurance, we will work with you to provide your eye care at an affordable rate.
