    Call our office

    Call (423) 243-3330 during office hours. Tell us whether you are a new or returning patient and the reason for your visit.

    Have your insurance handy

    We'll ask for your insurance information and let you know if your plan needs a referral or prior authorization.

    Come to the Erlanger Medical Mall

    Our office is on the second floor of the Erlanger Medical Mall, 979 East 3rd Street, Suite 240. Plan for one to two hours for a new-patient exam.

Before your visit

  • Photo ID and insurance card(s)
  • A list of your current medications
  • Your glasses, and your contact lens prescription if you wear contacts
  • Sunglasses — your eyes will likely be dilated
